PERSON CENTERED PLANNER

Perspectives is seeking a Person Centered Planner for our Self Directed Services program. Self Directed Services provide individuals with disabilities the opportunity to choose, manage and implement their own plan of support, with professional plan writers available to assist them.


JOB DESCRIPTION


The Person Centered Planner is responsible for creating the Individual Support Plan using Person Centered Thinking and working with the individual and their chosen network of support. Plans are submitted to the Department of Behavioral Healthcare, Developmental Disabilities and Hospitals (BHDDH). Person Centered Planners are contracted with the Sole Proprietors through the Self-Directed Program.

The Person Centered Planner will also:

  • Facilitate community mapping to match community supports to the individual
  • Facilitate pre-planning with the individual, his/her family and other allies
  • Facilitate a "planning event" that is respectful and positive
  • Facilitate the development of personal profiles
  • Develop personal plans that are community based and provide sufficient detail about how the plan will be implemented
  • Facilitate a review of the plan at least quarterly

DESIRED QUALIFICATIONS


  • Experience in writing Person Centered Plans for people with intellectual or developmental disabilities
  • Understanding the unique nature and strengths of each individual
  • Understanding community resources, community organizations and other natural supports
