Perspectives' Personal Assistance Services and Supports (PASS) program is a specialized consumer-directed service provided to Medicaid eligible children who live at home and have been diagnosed with significant physical, developmental, behavioral or emotional conditions.
In the PASS program, the family responsibilities include recruitment, training, management, and supervision of the PASS worker. Families have the flexibility and responsibility of managing their service hours on a weekly basis. The PASS agency Perspectives will support the family in the above areas as needed.
The PASS agency collaborates with the family to develop the PASS Care Plan. The PASS Care Plan contains child-specific goals that promote skill development both in the home and community environments, and focus to improve the child's ability to complete daily living skills, make safety and self-preserving decisions, and to participate in social roles and social settings.
The PASS agency support team is comprised of a Clinical Consultant and a PASS Coordinator who will work with each family to ensure that the service plans are appropriate for the child's needs, and make any changes that are necessary to foster child development and independence.
