PERSON CENTERED FACILITATOR
Perspectives Corporation is hiring a full-time Person-centered Facilitator to assist people and their chosen contributors to actively participate in developing the person’s whole life plan. The Facilitator is responsible for preparing, planning and writing meaningful person-centered plans with quality indicators in mind. The Facilitator monitors progress quarterly and adjusts plans as needed.
REQUIREMENTS
Minimal Qualifications
- Ability to prepare & engage people with intellectual/developmental disabilities in the person-centered planning process
- Experience with graphic facilitation
- Familiarity with: self-determination, community mapping, current plan formats (e.g. PATH), goal attainment
- Strong communication skills (verbal, written and face-to-face)
- Superior listening skills
- Excellent attention to detail and follow-through
- The ability to work autonomously and in collaboration with others
- High school degree or equivalent
- Valid driver’s license, clean driving record and insured vehicle
