Community Liaison

Perspectives Corporation is now hiring an Community Liaison to work in our Adult Services division.
OVERVIEW
The Community Liaison is Responsible for implementing person-centered strategies to support participants to increase meaningful community membership and inclusion, build social capital, and for promoting relationships with community members. Community life engagement may include volunteerism, continuing education, accessing the community to explore and/or engage in hobbies, areas of interest and/or personal goals, joining and participating in associations, clubs, civic and faith-based organizations.
Broad Areas of Responsibility:
Participating in agency activities, serving on committees, and communicating the agency’s values and principles to others; coordinating and monitoring effective supports and developing effective multidisciplinary teams; providing training and mentoring to new and existing employees; writing reports/updates on programming; caregiver contact.

JOB REQUIREMENTS
Minimal Qualifications
- High school degree or equivalent
- Demonstrates excellence in building partnerships with community organizations
- Experience in providing direct support to recipients of community human services
- Ability to establish rapport and relate sensitively to participants and their caregivers
- Strong verbal, written and face-to-face communication skills
- Ability to prepare & engage people with intellectual/developmental disabilities in the person-centered planning process
- Good computer skills
- Excellent attention to detail and follow-through
- High energy & motivation to help participants succeed
- The ability to work autonomously and in collaboration with others
- Valid driver’s license, clean driving record and insured vehicle
Desired Qualifications
- An Associate Degree or certificate in a human services field or related experience
- Experience with community mapping and/or relationship mapping
- Certification or proof of enrollment in Planning Alternative Tomorrows with Hope (PATH), Person-centered Thinking (PCT) Facilitator, Association of Community Rehabilitation Educators (ACRE) approved training and/or Supported Employment 101 (SE101)
